LFI Coordinator
Job Purpose:
Facilitate and coordinate the organization's continuous improvement of Learning from Incidents process by creating a learning mindset at all levels in the organization, ensuring that valuable insights are extracted from incidents, accidents, and near-misses, and that these lessons are effectively implemented to improve safety, efficiency, and overall organizational performance.
Minimum Education:
Bachelor's degree in Safety Management, Engineering, or relevant discipline.
Professional certification in incident investigation, process safety or safety management (e.g., NEBOSH, CSP).
Minimum Experience:
Minimum of 5 years of experience in safety management, incident investigation, or related field.
Proven track record in implementing and managing LFI programs.
Experience in facilitating root cause analysis and incident investigations.
Demonstrated ability to influence change and drive continuous improvement.
Familiarity with incident management software and databases.
Experience in developing and delivering training programs.
Accountabilities:
- LFI Process Management and Improvement:
Manage the end-to-end LFI process, from incident reporting to lesson implementation and impact measurement.
Develop, maintain, and improve the incident management system and database to ensure data quality and effective reporting.
Monitor and evaluate the effectiveness of implemented changes as per lesson from the incidents and the overall LFI program.
Lead the development and implementation of standards and policies related to LFI by driving the preparation and revision of standards, instructions and procedures as well as liaising with SMEs (Subject matter experts) to ensure sustainability of policies, guidelines and protocols with respect to incident investigation & reporting processes.
Ensure the implementation and improvement of the LFI reporting software through identification or consolidation of improvements, assisting with the evaluation of LFI software improvements and liaison with ICT for the implementation of the agreed improvements as well as testing of the changes before going live.
Prepare and issue flash notifications on incidents by conducting awareness campaigns and through issuing bulletins, general information and warnings re-incident investigations on the nature of priority incident and the immediate mitigating measures taken.
- Incident Investigation and Analysis
Facilitate incident investigations and root cause analyses by allocating resources based on incident severity and learning potential.
Ensuring that all incident investigations and associated actions are assigned and completed within the specified timeframes and that actions are closed timely to mitigate the risk.
Ensure compliance with the incident reporting, investigation and learning standard during execution of investigative process with adequate monitoring and analysis of the incidents to ensure proper governance.
Administer the functionality of the Consolidated Incident Management System, including review and correction of the incidents classification and data quality to ensure effective management of incidents.
Compare incidents and investigations to align with OASIS observations (behaviour-based safety observation) and ensure proper recording in the respective databases and close out. Analyze incident trends and patterns by comparing with behavioral safety observations when relevant to identify systemic issues.
- Communication and Dissemination
Create and disseminate LFI reports through appropriate communications channels including flash notifications to the right people at the right time in the right format.Prepare and present regular reports on incident trends, learnings, and program impact to senior management and leadership.
- Implementation and Coordination
Implement and track corrective actions and improvements through coordination with departments and stakeholders to ensure alignment with risk assessments and change management processes.
- Training and Development:
Develop and deliver LFI training programs to enhance incident response and analysis skills across the organization.
Support and influence leaders at all levels through coaching and skill-building to drive learning and behavioral change.
- Knowledge Management:
Maintain a knowledge base of incident patterns, root causes, and effective solutions to support ongoing learning and improvement.
